What do you have your controls set to? Other than default.
I have defense set to right click, block set to shift and the key on the side of my mouse (mb 4) weapon 1 and 2 set to scroll up and down, and the movement keys all switched to the right by one key because the keyboard is broken.
Default setting
Resolution set to 1024x768 (16 bit)
Crosshair Type 2
Right shift toggle

Left click shoot, middle button is block, right click is flip. Q is reload and thats about it.
right click = block, middle button + e = flip been playing like this for yrs
Left click = Jump
Right click = Shoot
E = Reload
W A S D = move
Space = Defense
mouse middle click = flip :).

2 configs
1: Q and E are replaced by 1 and 2 to ensure faster switching.
1 and 2 for medkits, nades, other secondary items.
Weapon 1 and 2 is scroll up and down.
Everything else is default.
I call this my LD config. Lol
2: Scroll up for sword
Scroll down for gun 1
Right click for for reload.
The rest is set with config 1 or default.

I learned to play like this, and this is how I played the most:
Right mouse button: Attack
Left mouse button: block
Shift : flip
(I got confused when the guy told me to change my controls around as a newbie, and it was too late by the time I realised it :/)
Changed a bit because of poor control customisation on private server, but that was it :)
