Reason: multiple death avoidance. only caught 2 times in recording but in my 2 other screenshots you can see that once he left the room, the game ended, then he joins back again.

For avoiding death you do have to provide a replay as the threshold for avoiding death is 3 times, and in your replay he did it twice as you've mentioned above.
Therefore, user has received a verbal warning and I've sent him an offline message about it.
