Boxes appear after you click the confirmation messaging saying you want to fix all the problems. I tested it on one and one 0 popup came up. Then I tested it on 5 and 5 came up. I guess I was impatient and didn't click all 5.2k popups?

When clicking on Fix Checked HiJackThis will, obviousily, fix the checked entries but doesnt prompt with a msg.
Might be a sort of protection of the dropper youve got there?
You should download some Anti Malware/Anti Virus tools and do a scan with em.
But install em once after you finished the scan, some Anti Virus or Anti Malware softwares hate eachother and will
fight the final battle on your pc.
The last thing to do would be making a backup of the important files and properly format the hdd to reinstall windows again.
But to be honest, i dont expect that much from Vista ^^
