1. Always include atleast two (2) screenshot showing their ping by pressing tab. Report may not be looked at if this is not included.
Usually around 40 ping, but alot of spike too.
2. No 10 seconds video, video must be a decent amount of time.

3. Take multiple video recordings if possible. Sometimes other videos display different results, especially if the person is decent at hiding it.
I have more recordings if you need them.
4. And of course, be sure to include the users name in the report.
