    I dont know anything about programming but is there a reason it can never be 100% accurate?

    It mostly affects shotguns because of its spread and the individual pellets.
    It's a terrible thing, I think, in life to wait until you're ready.
    I have this feeling now that actually no one is ever ready to do anything.
    There is almost no such thing as ready. There is only now.
    And you may as well do it now. Generally speaking, now is as good a time as any.

    It's all client sided, we don't know the exact reason.

    It would require a different implementation method to be 100% accurate. Dmg counter and antilead are best designed hand in hand. 99% accurate would be easy enough but that last 1% to get to 100% requires checks to be put in place and almost as much code as the first 99%. If you want it done right. Its good enough as is.
